How to File
Your corporate name, Doing-Business-As name , and premises address must be exactly the same on all documents. In Section 6 of the PW2, the contractor must respond to the HIC License question (Does the Work Permit applied for require an HIC license?). Do I need a home improvement contractor license in New York?
If you’re applying in person, you will have to complete an application and bring it to the Department of Consumer Affairs Licensing Center or the NYC Small Business Support Center. To apply online, you need a Department of Consumer Affairs account.
